Health Coverage Information in an Easy-to-Read Format

In 2007 Dean Health Plan began mailing a Health Insurance Benefit Summary (HIBS) form to members of employer groups. The HIBS form contains basic information about your group coverage and is intended to give you an easy-to-read overview of your coverage.

You will receive a HIBS form when your policy is renewed in 2008. If you have questions, call our Customer Service Department at 608-828-1301 or 800-279-1301.
