Take the Personal Health Assessment (PHA)—Your PHA is a powerful health assessment tool that can give you a clear picture of what you’re doing right, and suggest some ways to stay healthy. Just answer a few questions, and you’ll receive an accurate, confidential, and personalized report that includes:
- An intuitive, at-a-glance summary with prioritized results.
- A customized plan to reduce or eliminate any risk factors, should you have any.
- Interactive scenarios showing the impact of positive changes in your lifestyle.
Health Management Centers—Based on the answers you’ve provided in your Personal Health Assessment, Health Management Centers provide helpful information, resources, and tools to support you in your efforts to improve and maintain your health. You’ll receive the information you need now to achieve a healthy future.

Your
Personal Health Record (PHR)—Chances are, you’ve got medical records and information from doctors, pharmacies, labs, and other health care providers to keep track of. With your Personal Health Record, you can combine all of your fragmented medical information into one, accessible, secure location. That way it’s organized and easy to find when you need it. And, beginning this month—when you create your Personal Health Record, your claims are automatically added as they occur. You don’t have to remember to add them yourself.
Health Trackers—Chart your progress over time to help you stay motivated. Track blood pressure, cholesterol, body fat, and several other health factors. Or customize the tool by adding a new Health Tracker for additional data you want to track—like test results, number of push-ups, etc.
Symptom Checker—Click on where it hurts! The Symptom Checker is a head-to-toe tool that helps you determine how serious your symptoms are—and what to do about them.
Treatment Cost Advisor—Helps you estimate your costs for hundreds of common conditions—including procedures, tests, and health care visits, so you can plan and budget your expenses.
Hospital Advisor—Find the hospitals near you that are best equipped for the service you need. You can compare them based on their experience, cost, patient satisfaction, and other factors important to you.
