From primary care, to specialized care to dental care and more, CFCC has the services you need to keep your family healthy.

Something for Everyone

“Patients are excited to learn about all we have to offer,” says Carol Lau, FNP, associate director of Montefiore Medical Group CFCC. “We provide comprehensive primary and specialty care and offer a number programs that are difficult to find anywhere else.”

At CFCC, nearly 170 health professionals — physicians, nurse practitioners, midwives, social workers, nutritionists and health educators — work as a team to care for patients at all stages of life. Staff provide primary care, prevention services and specialty care in a number of areas including: allergy, dental, endocrinology, gastroenterology, obstetrics and high-risk pregnancies, gynecology, hematology, ophthalmology, podiatry, pulmonary, renal, rheumatology and surgery. When patients need services not available at CFCC, doctors refer them to specialists within Montefiore Medical Center.

Research and Unique Programs Enhance Care

CFCC physicians are on the staff of the Albert Einstein College of Medicine and many are involved in research, which allows patients access to cutting-edge treatments and clinical trials. As a teaching facility, the center is involved in training future healthcare providers. In addition, CFCC receives federal and state funding that supports unique programs that target patients’ specific needs. For example, some pregnant patients may participate in support groups that help them manage stress and their health during pregnancy, or provide financial assistance for prenatal care. Young children who visit the pediatrician may receive a free book to help them develop a love of reading and prepare them for school. Patients with diabetes learn how to better manage their disease with exercise and proper nutrition. Those living with HIV get not only medical care, but also support and information.

“Patients choose CFCC because of the exceptional medical care they receive in one convenient location,” Ms. Lau says. “And we have patients on our advisory board, so they have a say in the type of care and services we provide.”
